Commonwealth Home Support Progam (CHSP)
The Commonwealth Home Support Program (CHSP), is a funded by the Dept of Health, Disability and Ageing, that provides entry-level support for seniors, who need a little extra help to stay independent, in the comfort of their own home.
This level of support reflects nearly all of the Prime Care Partners' key in-home care services, such as help with daily personal care, domestic assistance which includes shopping for and laundry services, assistance with meal preperation and in home respite care.
As this is an entry-point level, most people utilising the funding, may only need a few services to help them to 'get back on their feet' and achieve their goals, or may use it to provide extra care, to an existing service (eg: provided by the Prime Care Partners team).
As assessments for this program can often take time to understand and navigate, a Prime Care Partners Care Advisor, can help you through the CHSP process, to simplify and streamline, wherever possible, and to liaise with all of your existing home and health care professionals, to ensure a holistic and integrated approach - to your support needs.
